What You Will Need to Apply P

Please take a moment to read this information before you begin your report.

To file the report, you will need to use the same email address and password that you used to file the application requesting the funds. This will connect your grant application with this report, allowing the report to auto-fill information and expediting the process for you.

Although you may stop and save your work as you file the report, please be sure to gather the following information before you start:

  • Any collateral files you wish to submit to support the report and the requirements of the contract (limit of 8 files, most formats acceptable)

  • Program or project budget

Information about your project, including:

  • General information (see contract for project name, contract number, etc.)

  • Short narrative of the overall benefits of the grant to the organization and/or the community (limit of 300 words)

  • Activities (limit of 300 words)

    • What activities were supported by this grant and how did it serve your organization’s mission?

    • Were there any collaborations with other organizations?

    • Were you able to leverage any volunteer resources?

  • Impact - How did the activity supported by the grant impact the community? (limit of 300 words)

    • Key metrics

    • Demographic information

    • Outcomes, including an example of how the grant contributed to the results

  • Anecdotes or Stories - Share any story of someone’s life that was impacted by this grant (limit of 300 words)

  • Sustainability Plan (limit of 300 words)

  • How many persons benefited from this grant

  • Venue(s)

  • City Council District (unless a city-wide program/project)

  • Additional optional comments, if necessary (limit of 300 words)

  • Short summary - This is something we might share with media or donors regarding your organization’s benefit from the Moody Fund for the Arts (limit of 100 words)

  • Organization’s contact information (may be auto-filled from application)

  • Primary person’s contact information (may be auto-filled from application)

Once you are ready to begin, please start with the application form below. Please be sure your email server will allow you to receive emails from noreply@mail.smapply.io and from @surveymonkey.com. If your email address has changed and you cannot use the original one, you will need to go through the email verification process as you did when you first created the account. You will not be able to submit the report unless your email is verified.
